If you’ve just been released from the hospital or are unable to go out, an in-home nurse can offer treatment to you. A home care package is a coordinated package of care and services to help you live independently in your own home for as long as you can.
If you are eligible for the government-funded Home Support Program, an assessor will visit your home to assess your requirements, after which you will be placed on a wait-list.
There are four levels of Home Care Packages to help meet the different levels of care needs. During your aged care assessment, your assessor will discuss your current care needs and determine the best level to meet them.
The four categories are:
LEVEL 1 – For basic care needs. Provides approximately 2 hours of home care services a week
LEVEL 2 – For low-level care needs. Provides approximately 4 hours of care a week
LEVEL 3 – For intermediate care needs. Provides approximately 9 hours of care a week
LEVEL 4 – For high-level care needs. Provides approximately 13 hours of care a week
You’ll first need to complete a referral to My Aged Care. The Aged Care Assessment Service (ACAS) will then evaluate yours or your loved one’s needs for funded support at home. The assessment will then be entered into the My Aged Care system, where you’ll be approved or one of four levels of funding.

If you’re eligible, you’ll be allocated a Home Care Packages based on the level of care you need.
There are a maximum amount of packages available in Australia at one time. Once you’ve been assigned a Home Care Package, your funds will be made available through a service provider of your choice. Your service provider will work with you to select, plan out and schedule a range of services to suit your needs and lifestyle.
